MINIMUM SUSPENSION LIST

Coaches, managers and team officials are responsible for ensuring that all applicable minimum suspensions are served.

CMHA Suspension Form

Use the Blank Suspension Form when a player or team official receives a suspension.

Instructions:

• Complete all applicable sections of the form.
• Identify the player or team official, team, division, game date, infraction and suspension.
• Refer to the 2026–2027 NOHA/OHF Minimum Suspension List above to confirm the applicable suspension.
• Please email the completed form to [email protected] within 24 hours of the suspension.
• A completed form must also be emailed for each game in which the suspension is served.
• Ensure all required suspension games are properly recorded.
• Keep a copy of the completed form for team records.
• Contact the Director of Team Operations if clarification is required.
