🏥 Hockey Canada Injury Report
A Hockey Canada Injury Report must be completed when an injury is sustained by a player, team official, game official, spectator or other person during a sanctioned hockey activity.
Important:
Claims must be presented within 90 days of the injury date. The Injury Report must be completed in full.

Submitting an Injury Report
Once the Hockey Canada Injury Report has been completed, please email the completed form to both:
