CMHA Travel Permit Request

All Coniston Minor Hockey Association team managers requesting permission to travel to a tournament, exhibition game, jamboree, or other hockey event must complete and submit this form.

IMPORTANT — SUBMISSION DEADLINE

All travel permit requests for tournaments must be submitted a minimum of 60 days prior to the tournament start date to allow sufficient time for review, processing, and approval.

Managers are responsible for ensuring all information is accurate and submitted within the required timeframe.

Completed requests will automatically be forwarded to the CMHA Secretary and Vice-President for review. A confirmation copy will be emailed to the submitting manager.

Please note: Submitting this form does not constitute approval of a travel permit. Teams must receive the required authorization before participating in the event.
