Player Play-Up Evaluation Form
Purpose: This evaluation is used to assess whether a player demonstrates the
hockey ability, physical readiness, maturity, coachability and developmental readiness
necessary to be considered for participation in the next age division.
Important: Participation in an evaluation or a positive evaluator recommendation does not automatically authorize a player to play in a higher division. Final placement remains subject to applicable CMHA, league and governing-body requirements and approvals.

CMHA may consider player development, safety, readiness, team composition and the effect of the requested movement on both the player’s age-appropriate division and requested division when reviewing a request.
